Athletic Director News

 

Fletcher is proud to offer several sports opportunities for our 7th and 8th grade students.

6th grade students may try out to be team managers. Please see the individual coaches for this information. Managers must follow the same physical and GPA guidelines as all athletes.

Information about tryouts is available from the coaches. Posters are put up on the Sports Page bulletin board in the cafeteria and announcements are made on the morning TV show. Also, information is released in our school newsletter, THE WAVE, each quarter.

 


In order to participate - INCLUDING TRY OUTS AND MANAGER POSITIONS - with any team, a student MUST meet the following requirements:

* 2.0 GPA (using report card grades) based on the following:

    ---1st time entry into 7th grade, eligibility is based on promotion status for the first nine weeks only

    ---1st nine weeks for 8th graders (or repeating 7th graders), eligibility is based on the previous year’s cumulative GPA

    ---At all other times during your 7th or 8th grade year, you must have a 2.0 GPA from the previous nine weeks

     ---Grade recovery will not change your eligibility during the quarter. No exceptions. Please use the progress reports you get from teachers during the nine weeks to stay on top of your grades.

* CURRENT PHYSICAL on file with Ms. Phillips in room 316.  Physicals are good for 365 days.

* PROOF OF CURRENT MEDICAL INSURANCE on file with Ms. Phillips in room 316.

 

* CONSENT AND RELEASE FROM LIABILITY CERTIFICATE on file with Ms. Phillips in room 316.

* SPORTSMANSHIP CONTRACT on file with Ms. Phillips in room 316.

- Appropriate age guidelines (no more than two years of eligibility are given starting the first day of the students' first term in grade seven, and no student who turns fifteen BEFORE September 1st, of the current school year will be allowed to participate)

All paperwork is available from  Ms. Phillips in room 316, and must be returned to Room 316.          

                **Do not take paperwork to the office, or turn it in to a teacher**


Special Note about Physicals:

PLEASE fill out the form COMPLETELY!! I cannot stress this enough. Double-check for the following:


Complete name and address

Emergency Contact Information

Parent/Guardians need to sign twice on the front of the form!

Athlete must sign on the front of the form

 

Physician must sign and date the back of the form.  Physical forms without a physician’s signature and date are considered expired.

We will only accept the “DUVAL COUNTY SCHOOLS PREPARTICIPATION ATHLETIC SCREENING FORM”.   County policy prohibits acceptance of FHSAA or "PHYSICAL EDUCATION" RELEASE FORMS”  Use the link below to print a copy of the current year DUVAL COUNTY SCHOOLS PREPARTICIPATION ATHLETIC SCREENING FORM.

Please do not trust that your doctor has the correct form. Your doctor deals with sports physicals for a variety of reasons. Please use the link above to download the form or send your child to Ms. Phillips in Room 316. Also, remember physicals are good for 365 days. If a physical expires mid-season, that student is benched until a new physical is completed on the appropriate form and turned in to Ms. Phillips.

Special Note about Medical Insurance Information:

If you do not have insurance and wish for your child to participate, you need to purchase student insurance. This information goes home during the first week of school with your child. You can also get this paperwork from the guidance office. Student insurance packages start at approximately $11.

Parents, please understand that a lot of paperwork is involved FOR THE BENEFIT AND SAFETY of ALL our athletes. In order to make this process less painful, please adhere to the following guidelines:

- Do not drop physicals off in the office. Send them WITH your student to Room 316. Do not give it to anyone but Ms. Phillips.

- Please keep your own copy of the physical. You may need it for other activities or events outside of FMS. Due to budget constraints, we cannot make copies for you.

- Get physicals done in advance!!! Please DO NOT wait for the day of tryouts!  If your physical is not complete or turned in before try-outs, you may have to wait for the next day.

- We CANNOT make exceptions for students for any reason. It is my job to protect the students and the school by adhering to county policy. It is your job to have the correct paperwork for your athlete. It is the athletes' job to bring the paperwork to Ms. Phillips. If we all do our jobs, we'll have a great season!
